2013 ST. LOUIS KICKOFF PROCEDURES  **IMPORTANT** CONSENT AND RELEASE FORMS  Don't forget your Consent and Release forms!  FIRST Consent and Release forms are required for student participation at all FIRST events. Participation at any event is not guaranteed until a completed FIRST Consent & Release Form is submitted for every team member (mentor and student). Team mentors and students attending the FIRST Events must complete the FIRST Consent and Release form each season. Student Team members under 18 must have a parent or legal guardian's signature on their form. If students or mentors have completed the consent form electronically, you do not need to bring a printed copy of the consent form for those team contacts, as the Team Roster indicates the form was completed online. If a student or mentor chooses to complete the form on paper, bring the signed paper consent form with the Team Roster to the Kickoff event.  The hard copy paper FIRST Consent and Release form can be downloaded at: http://www.usfirst.org/roboticsprograms/frc/content.aspx?id=8128  Registration/Kit of Parts Receipt 1.  ÂYou will be given your Kit of Parts (KOP) receipt at registration. For those of you attending the Kickoff for the first time, this receipt confirms that your team can pick up a KOP.  2.  Â**IMPORTANT** Hold on to this receipt until the Kickoff ceremonies are over. We do not have duplicates of these receipts, so do not lose yours.  3.  ÂBring the receipt with you to Kit Handout.  Kit Pickup 1.  ÂThe KOP's will be distributed from the Exploradome Lobby, following the Kickoff ceremonies. 2.  ÂYou will need to transport the KOP from the Exploradome to your car. A luggage dolly (150 lbs minimum) can be a big help with transporting your KOP.   Kit Handout 1.  ÂKit Handout will take place following the Kickoff event. 2.  ÂYour KOP receipt is designed with checkboxes for each component. Make sure the Kit of Parts you are collecting match with the checkboxes on the receipt. 3.  ÂAs you proceed through the handout line, check off each item you receive in the appropriate box. 4.  ÂSign and print on the line provided. 5.  ÂTurn in the KOP receipt to the designated collector(s) before you leave the pick up line to head home. (It is required that an adult mentor of the team sign and return the KOP receipt to the FIRST crew member handling kit distribution at each Local Kickoff site. Kits will not be released to student members of the team.)  For additional details on the Kickoff, go to Local Kickoff Facts . We urge you to read through this information before coming to Kickoff. If you have any questions please contact Team Support at 800-871-8326, dial 0 or email us at frcteams@usfirst.org.
